YEAR END TAX CONTRIBUTION STATEMENTS FOR 2010To receive your year-end contribution statement, please complete a REQUEST FORM. Completely fill out the form and place it either in the box located next to the forms or drop it off at the parish office. As your form requests, we will either mail or have your statement available to pick up in the Church office during weekday business hours or during Sunday office hours from 8:30 a.m.—1:30 p.m. Statements will be available beginning January 31, 2011. Statements will automatically be sent to those who have contributed a one time $250 or more donation. Statements will also include Bishop Annual Appeal contributions during 2010.
